What is a Patient Portal?
A patient portal allows patients access to information about healthcare services they receive from CMC through our secure website.

* View/Print your personal Patient Health Record Summary

How do you use the Patient Portal?
You access the Portal from the link on this page. You will find Portal Authorization forms on our "Forms" page. Please complete the forms and submit them to a CMC caregiver. We will mail your user ID and password. If you have signed up for Portal access and have not received your user name and password, please call CMC at 541-548-2164.
